Requirements
Must:
- A passion for empowering individuals to achieve their goals
- Strong leadership skills to foster team culture and support staff development
- Experience in monitoring services within community-based settings
- Proficiency in using Electronic Medical Records (EMR)
- Familiarity with budget management and financial planning
- Excellent communication skills for interacting with individuals, families, and case managers
- Ability to adapt to changing schedules and provide on-call support as needed
- Knowledge of policies and regulations governing service delivery in the industry
- Proven documentation skills for incident reports and service records
Responsibilities
- Cultivate and maintain a positive team culture through active engagement
- Conduct monthly team meetings to address immediate needs
- Provide coaching and professional development opportunities for staff
- Be available on-call to cover scheduling gaps and respond to emergencies
- Oversee resource management, including vehicle utilization and budget adherence
- Facilitate communication between individuals served and their Representative Payee Coordinator
- Ensure timely scheduling and follow-up of appointments and medication check-ins
- Serve as the main communication link with the Medicaid Community Based Care Manager
- Develop personalized plans in collaboration with the Interdisciplinary Team
- Conduct regular assessments and updates to ensure service appropriateness
- Maintain accurate documentation of all interactions and service modifications
- Collaborate to enhance community partnerships and promote services offered
