Requirements
Must:
- Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in the state of application.
- Proficient oral and written communication skills in both English and Spanish.
- Basic technological skills, including familiarity with electronic health records (EHR).
- Strong organizational abilities and documentation skills with a focus on regulatory compliance.
- Capability to provide in-person services at designated facilities, with travel up to 45 minutes.
- Physical ability to walk, stand, and navigate between patient rooms and offices throughout the day.
- Full vaccination and booster may be necessary for certain facilities.
Responsibilities
- Conduct individual and group therapy sessions for patients in subacute or skilled nursing environments.
- Assess, diagnose, and provide treatment for various emotional and behavioral health issues, including depression, anxiety, adjustment disorders, and cognitive decline.
- Create and execute evidence-based treatment strategies tailored to individual patient requirements.
- Collaborate with facility personnel, interdisciplinary teams, and family members to enhance patient care.
- Ensure clinical documentation is accurate and timely in line with regulatory standards using our clinician-designed electronic medical health system (EHR).
- Track patient progress and modify treatment plans when necessary.
