Requirements
Must:
- Graduation from an accredited college or university with a bachelors degree in social work or a related field.
- At least three years of professional experience in mental health services.
- Experience working with diverse communities.
- Valid professional license and/or certification, along with a valid drivers license in good standing.
- Strong organizational skills to manage multiple projects and individuals concurrently.
- Excellent interpersonal skills and the ability to work effectively in a collaborative team setting, exhibiting fairness, respect, and inclusivity.
- Proficient oral and written communication skills.
- Computer literate with expertise in database and reporting tools, including Microsoft Word, Excel, Access, and PowerPoint.
Responsibilities
- Provide ongoing in-person and phone communication with families, CP&P, and relevant stakeholders.
- Communicate honestly, respectfully, and with cultural sensitivity.
- Utilize Motivational Interviewing techniques consistently.
- Employ a family-centered methodology in interactions.
- Implement persistent and innovative outreach strategies to encourage family participation in services.
- Conduct initial meetings in a private setting to foster trust and understand family needs.
- Gather essential information through structured processes and complete necessary assessments.
- Create and regularly update service plans based on ongoing family assessments.
- Facilitate discussions among families and their support teams about needs, challenges, and definitions of success.
- Collaborate with CP&P and community organizations to enhance support networks.
- Convene or participate in team meetings to assess progress and adjust services as needed.
- Advocate on behalf of families and support them in self-advocacy efforts.
- Promote broad advocacy initiatives and provide educational support for skill development.
- Assist families in formulating and implementing comprehensive service plans.
- Research and connect families with suitable community resources and supports.

We provide health benefits, on-site childcare, and a 401K plan, with a competitive salary ranging from $65,000 to $75,000 based on experience.
