Requirements
Must:
- We require 7+ years of hands-on production experience with React and TypeScript.
- We require 7+ years of production experience with Node.js, Express, or Next.js.
- We need advanced PostgreSQL expertise, including data modeling and performance tuning at scale.
- We require substantial AWS production experience across EC2, RDS, S3, Lambda, CloudWatch, and KMS.
- We require demonstrated experience building HIPAA-compliant applications.
- We need production experience integrating Twilio Voice and SMS.
- We require production-level LLM API integration experience with Anthropic Claude, OpenAI, or similar tools.
- We need experience implementing role-based access control with Auth0 or an equivalent solution.
- We require experience supporting systems with 10,000+ concurrent user records.
- We need the ability to independently deliver a production MVP within 60 days.
- Prior experience with real-time transcription APIs such as Deepgram is a plus.
- Background in healthcare SaaS or digital health is preferred.
- Familiarity with HEDIS quality measures or Medicaid billing is a bonus.
- Experience as a first engineer, sole engineer, CTO, or technical lead at an early-stage startup is advantageous.
- We prefer candidates who are based in or able to travel periodically to St. Louis, Missouri.
Responsibilities
- We are building the COUNSELMEDS production platform from a fully designed and functional interactive demo.
- We will have you work directly with our founder and independently own key architectural decisions.
- We expect you to develop a HIPAA-compliant clinical operations platform for licensed pharmacists managing Medicaid member caseloads.
- You will create the pharmacist workflow dashboard, including telehealth call integration.
- You will implement AI-assisted clinical note generation from call transcripts.
- You will build an automated patient SMS check-in system with biometric response interpretation.
- You will develop medication adherence tracking for large-scale member populations.
- You will create a billing readiness dashboard to support clinical claims management.
- You will design and deliver HIPAA-compliant AWS infrastructure with complete audit logging.
- You will implement secure role-based access control for multiple user roles.
- You will move quickly, make decisions with minimal supervision, and deliver a working production platform within 45 to 60 days.
Description
We are building COUNSELMEDS, a HIPAA-compliant clinical operations platform for licensed pharmacists serving Medicaid populations. This is a senior full stack role with direct access to our founder and significant ownership over the products architecture and delivery. We have a fully designed interactive demo, along with detailed technical specifications, data models, and integration documentation available to shortlisted candidates under NDA. The work is hybrid remote in St. Louis, Missouri, and we offer competitive hourly pay ranging from $75 to $150. Our process includes an intro call, NDA execution, a technical architecture discussion, a paid take-home project, and a final interview, with a decision made quickly after the final round.
