Requirements
Must:
- Relevant work experience or a bachelors degree
- Dependability and ability to uphold confidentiality and professionalism in compliance with HIPAA
- Valid drivers license and a clean driving record
- CPR certification required or the capability to obtain it within 3 months of hire

Responsibilities
- Evaluate participants strengths and challenges related to ADLs and IADLs through an independent living skills functional assessment
- Work alongside the Case Manager to formulate an ILST plan detailing goals and the necessary steps to reach them
- Offer tailored coaching and training in key areas like daily routines, household management, budgeting, bill payment, transportation use, and other skills vital for independent living
- Attend participant meetings as scheduled by the Case Manager
- Help organize and implement program activities, including social and educational events
- Facilitate group life-skills sessions
- Assist in crafting monthly structured programming
- Accompany participants on community outings or appointments, with a willingness to drive a company-provided vehicle
- Carry out other assigned tasks as needed
Description
We are dedicated to providing instruction and support to tenants in mastering Activities of Daily Living and Instrumental Activities of Daily Living. Our mission focuses on fostering independence and community living skills through collaboration with our program team. This full-time position entails working 75 hours every two weeks, including two Saturday shifts monthly. We offer a competitive hourly wage ranging from $18.50 to $19.60.
