Service navigator with tech support skills

- Conduct assessments and evaluations of family needs according to program guidelines, including analyzing presenting issues for appropriate service determination - Offer personalized support to school families referred by staff, assisting them in overcoming barriers and addressing their specific needs - Collaborate closely with the site coordinator to create achievement plans tailored for families - Partner with families to help them meet their goals and objectives, such as scheduling appointments and applying for public assistance - Work alongside the identified families, Team Leader, and school administrators to tackle issues related to attendance, tardiness, and behavioral concerns - Collaborate with the School Team Leader and school administrators to facilitate support and referrals for students and families - Provide assessment, strategic planning, referrals, advocacy, and collateral contact efforts on behalf of clients - Maintain regular communication with clients through meetings, phone calls, home visits, or other necessary methods - Establish connections with external community resources to assist families - Collaborate with school administration and team members to monitor critical data for measuring project success - Ensure timely and accurate documentation and record maintenance as per agency, statelicensing, and other regulatory requirements - Engage in regular staff meetings with school leadership and other staff to connect families needing further assessment and resource linkage while reviewing ongoing cases - Foster professional, collaborative relationships within the agency, community, and school environment
